Many of our patients have been told their tests are normal, yet they still do not feel well. They are tired of repeating their story, not being heard, and not getting a plan that truly moves them forward. Tantien Integrative Medicine is a physician-led practice in Branford, CT that combines integrative medicine, clinical experience, and genomics-informed care to build treatment around the person you actually are.
How Care Works at Tantien Integrative Medicine
1. Schedule a free 15-minute consultation to discuss your goals and confirm practice fit.
2. Complete a 90-minute comprehensive initial consultation with Dr. Tavernier.
3. Receive and complete a 3x4 Genetics at-home test kit (cheek swab, mailed to you).
4. Return for a 60-minute genetic blueprint review — Dr. Tavernier walks through your results in full.
5. If it is a mutual fit, enroll in an annual membership tier appropriate to your care complexity.
6. Receive a fully individualized, genomics-informed integrative care plan.
Practice is limited to 100 patients. Initial consultation package: $1,775. Annual membership tiers from $4,200 to $6,200. Cash-pay only. Payment in full is due at the time of scheduling. This is a genomics-informed integrative medicine practice. Each patient’s care plan is built from their health history, lifestyle, risk factors, and the results of a 3X4 Genetics clinical test, which is completed at home after scheduling and payment received. Because this testing is a core part of the practice model, this practice is not the right fit for patients who do not wish to participate.

The Initial Consultation Package
Everything begins with a deep, comprehensive understanding of who you are — your health history, your genetics, and your goals. This three-part package is designed to determine whether our practice is the right fit for your ongoing care.

Comprehensive Integrative Consultation
An unhurried, in-depth visit where we sit down together and truly listen. This is not a ten-minute appointment — it's the beginning of understanding the full picture of your health.
Complete medical, family, and surgical history review
Current medications and supplement assessment
Lifestyle assessment covering nutrition, activity, sleep, stress, emotional well-being, social connections, and environmental exposures
Discussion of your health goals, values, and vision for wellness
Complete physical examination as clinically indicated
Initial identification of root-cause contributors to your health concerns

Genetic Test Performed at Home
3x4 Genetics Clinical Genetic Test
After your initial consultation, a 3x4 Genetics test kit will be sent to the address on file. Your results reveal the genetic variants that shape how your body works — from nutrient metabolism and inflammation to hormone balance and stress resilience.
Analysis of key genetic variants across multiple biological pathways
A personalized report that serves as your biological blueprint
Interpretation in context — your genes inform your care, but they do not determine your destiny

Approximately 60 minutes
Follow-up & Genetic Blueprint Review
Once your results are ready, we meet again. Dr. Tavernier walks you through your genetic report in detail, explains what your unique variants mean for your health, and identifies the areas of greatest clinical importance.

If we're the right fit, a membership opens up your care journey
If it's mutually agreed that this practice is a good match for your ongoing care, we'll invite you to enroll in a yearly membership at the tier appropriate for the complexity of your health needs. Your personalized, genomic-informed integrative care plan is developed during your first membership appointment. Dr. Tavernier limits her membership panel to 100 patients — ensuring every patient receives the time and attention they deserve.
Your membership allows for ongoing reassessment, clinical reevaluation, targeted laboratory testing when indicated, and continued lifestyle and supplement recommendations. Mind-body-spirit practices, including meditation and energy healing approaches, may also be incorporated when desired as part of your broader healing process.

We work this way deliberately: it means your slot is fully protected, Dr. Tavernier can prepare for your specific history before you arrive, and there are no billing surprises after the fact. IMPORTANT INFORMATON
What to know before your first visit
Because this is a small, physician-led practice, we're transparent about how we work so you can decide whether this model truly supports what you're looking for in your healthcare.
Cash-Based Practice
This is a direct-pay practice. We do not bill insurance. Payment is due at the time of scheduling and your appointment is confirmed once payment is received. If requested, we can provide a superbill for you to submit to your insurer for possible out-of-network reimbursement.
Cancellation Policy
We require at least 72 hours' notice for cancellation or rescheduling. As a small practice, each consultation slot represents a significant commitment of provider time. With adequate notice, your payment will be applied to a rescheduled date.
Medicare Beneficiaries
Dr. Tavernier has opted out of Medicare. If you are a Medicare beneficiary, a separate Medicare Private Contract is required before services can be furnished. Please contact us to discuss this before scheduling.
Not a Primary Care Replacement
This practice provides integrative medicine services and does not serve as your primary care physician. You should maintain a separate primary care provider for preventative services and health insurance for hospital-based care and emergency services.
Genetic Testing Considerations
This Practice's integrative care model is built on your life story and 3x4 genetic test results. The genetic test is a simple at-home cheek swab kit mailed to your home — you collect the sample yourself and return it in the prepaid packaging provided. Because we develop your care plan based on an integration of your genetic data with your life story, patients who do not wish to participate should not schedule with this practice.
Telehealth Available
Follow-up visits may be conducted via secure telehealth when clinically appropriate and mutually agreed upon.
