This is a genomic-informed integrative medicine practice: Every patient’s care plan is built on their unique life's story and the results of a 3x4 Genetics clinical genetic test – a simple at-home swab mailed to you after scheduling. Your genetic test results are tused in conjunction with your history, lifestyle, and risk factors to develop a distinctive care plan. If you do not wish to participate in genetic testing, this practice is not right for your care.

Everything begins with a deep, comprehensive understanding of who you are — your health history, your genetics, and your goals. This three-part package is designed to determine whether our practice is the right fit for your ongoing care.
Comprehensive Integrative Consultation
An unhurried, in-depth visit where we sit down together and truly listen. This is not a ten-minute appointment — it's the beginning of understanding the full picture of your health.

3x4 Genetics Clinical Genetic Test
Once you schedule your initial consultation and payment is received, a 3x4 Genetics test kit will be sent to the address on file. Your results reveal the genetic variants that shape how your body works — from nutrient metabolism and inflammation to hormone balance and stress resilience.

Once your results are ready, we meet again. Dr. Tavernier walks you through your genetic report in detail, explains what your unique variants mean for your health, and identifies the areas of greatest clinical importance.

If we're the right fit, a membership opens up your care journey
If it's mutually agreed that this practice is a good match for your ongoing care, we'll invite you to enroll in a yearly membership at the tier appropriate for the complexity of your health needs. Your personalized, genomic-informed integrative care plan is developed during your first membership appointment. Dr. Tavernier limits her membership panel to 100 patients — ensuring every patient receives the time and attention they deserve.
Your membership allows for consistent reassessment and clinical reevaluation with targeted laboratory testing, continued lifestyle management, and recommendations for specific supplements as clinically indicated. Additionally, mind-body-spirit techniques — including meditation and energy healing practices — will be provided and taught as desired, supporting your whole-person healing journey from the inside out.

What to know before your first visit
Because this is a small, physician-led practive, we're transparent about how we work so you can decide whether this model truly supports what you're looking for in your healthcare.
Cash-Based Practice
This is a direct-pay practice. We do not bill insurance. Payment is due at the time of scheduling and your appointment is confirmed once payment is received. If requested, we can provide a superbill for you to submit to your insurer for possible out-of-network reimbursement.
Not a Primary Care Replacement
This practice provides integrative medicine services and does not serve as your primary care physician. You should maintain a separate primary care provider for preventative services and health insurance for hospital-based care and emergency services.
Cancellation Policy
We require at least 72 hours' notice for cancellation or rescheduling. As a small practice, each consultation slot represents a significant commitment of provider time. With adequate notice, your payment will be applied to a rescheduled date.
Genetic Testing Considerations
This Practice’s integrative care model is built on your life story and 3x4 genetic test results. The genetic test is a simple at-home cheek swab kit mailed to your home — you collect the sample yourself and return it in the prepaid packaging provided. Because we develop your care plan based on an integration of your genetic data with your life story, common patients who do not wish to particupate should not schedule with this practice.
Medicare Beneficiaries
Dr. Tavernier has opted out of Medicare. If you are a Medicare beneficiary, a separate Medicare Private Contract is required before services can be furnished. Please contact us to discuss this before scheduling.
Telehealth Available
Follow-up visits may be conducted via secure telehealth when clinically appropriate. Telehealth is not appropriate for emergency or urgent situations. Standard text and email are not considered telehealth under Connecticut law. Secure telehealth visits are conducted through our designated platform so your conversations remain private and protected.
